For patients receiving Hospital Medical Care – First Name, Middle Name, Last Name, Personal Identification Number (PIN), Address, Phone Number, Guardian Information (Name and Phone Number); Detailed medical condition of the patient; Copy of health booklet; Medical Record containing patient examinations – Medical History; Discharge Summary
4. For clients and partners – First Name, Middle Name, Last Name, Personal Identification Number (PIN), Address

The National Health Insurance Fund (NHIF), health insurance funds, regulatory authorities conducting compliance checks, judicial authorities and experts, other state authorities (police, investigation, prosecution, State Agency for National Security, Commission for Counteracting Corruption and Forfeiture of Illegally Acquired Assets, and others), insofar as there is a legal obligation to provide such data
2. IT service providers for the purposes of development and technical support of the systems of “SOBAL PENTAGRAM” Ltd.
